  Actions

  Planting a frag grenade directly on top of an emergence hole will work. Just be careful with your Actions Points. I accidentally killed a roster member because once I planted the frag, I didn't have any moves left to run away from the hole.

  I can confirm you can get this by planting a frag grenade on top on an E-Hole. It doesn't matter if your soldier can get out of the blast radio and/or if he/she is killed by your own grenade. If you are not playing on Ironman mode, you can just reload a previous checkpoint after you get this achievement the kamikaze way.

  Just to add to MCmichaelD's solution is that the best place to do this early in the campaign is in Act 1 Chapter 4 after you reach the rig. You will get a quick little cutscene of Sid saying something about the Grubs tunneling under you and then you will have a turn free of interference to get this.

  Just to add to this.
  A few solutions suggest the earliest this can be done is act 1 chapter 4.
  It can actually be done on act 1 chapter 2.
  
  Complete the first wave of enemies and position a gear with grenade/mine capability to the top left of the area.
  Near the gears rig tank.
  An emergency hole will appear up here when the second wave appears.
  Just plant a mine on top of it.
  Doesnt matter if you die from the explosion as the emergence hole appears just after the checkpoint.
  just reload.
