  I used Mikayla and 3 support units to get this. You want to make sure your support units have the empowered skill and that Mikayla has the Alpha and Omega skills. I did this against a pack of wretches as they don't hide behind cover so the hit probability is high, and that you don't need to waste points moving as they should all be in range from the same position. At the start of the turn use all 3 empowered skills on Mikayla. If you have empowered 1 you get 1 AP per boost, and if you have empowered 3 you get 2 AP. Just have Mikayla shoot at the wretches. The shots don't even need to hit.
  
  If you have at least empowered 3 you can just reload normally after your first 3 shots. If not, use the fast fingers skill on the third shot to automatically reload.

  I can confirm that you can get it really easy with the Epic Sniper weapon mod that reduces two ammo for +60 damage. With one bullet you trigger both alpha and omega automatically, reload, and do another shot to use your three actions. You do not have to connect the shots. I missed the first and hit the second.

  I got this when I'd swapped to the pistol part way through a turn, so you could probably also do a bit of setting up with leaving one bullet in there etc

  it also helps if you choose a mission with a modifier that reduces your magazine size by three, that way you only have to shoot twice and can do it without needing to use multiple support units.

  I did this on the very first mission of chapter 3.
  
  Mikayla: has the Alpha and Omega skills (obviously) and a piece of gear with the Blitz skill, higher chance the better. She has a chance to gain 1 AP if not firing for a turn.
  
  Gabe: Empower Lv3 and Lock and Load.
  
  Halfway through about 10 wretches spawn. Use your team to kite them until there are 6 or more in Mikayla's line of fire and Blitz procs, she'll have 4 AP. Gabe Empowers Mikayla, Mikayla empty the mag, Gabe's Lock and Load, Mikayla empty the mag. Done.
  
  Also a decent place to attempt both of the Heavy achievements I assume (I had the wrong loadout but wretches are easy pickings.)
