You notice the most that they changed the skill tree and cyberware (builds don't matter in this game). Stuff like changed police behavior and pedestrian behavior didn't really matter in my experience, they were never game breaking to begin with and I never cared. And cars still drive like shit, as in most open world games.
Beside that it plays pretty much the same overall. I played it a few weeks after release on PC and beside some floating objects, the cars crashing the wall in the city center, and one visual glitch in a car on mission that is still there, I experience none of the issues so prominently shown in videos in multiple 100% runs.
Recently played it on xbox again with the DLC. Beside the small additions, changed skill trees, some polish and QoL stuff, the overall game experience was pretty much the same now on console as my first PC playthrough.
It's a great game, one of the best I played and one of the few games I 100% more than once. Love the storytelling and Johnny. The Johnny thing carried the game. You could say Keanu carried it.
The DLC was a letdown, just a generic spy thriller - wished they continued with the Peralez story. I would never replay the DLC (had one or two good parts though), but the main game will for sure get a another playthrough from me.
