The aim of the game is to walk down the corridor and spot anything out of place or different from usual. There are 31 anomalies in total, and you need to experience all of them for an achievement. I’ve listed the anomalies in location order of the hallway to keep it as clear as possible. Follow the list and when you experience an anomaly, turn around and backtrack down the corridor and around the corner. If nothing is out of place, then simply continue forward and around the corner. The number on the large yellow exit sign will increase with each correct choice, or reset to Exit 0 upon a wrong choice. Once it hits Exit 8 and you go up the stairs, you’ve beaten the game. The game will allow you to run through the loop three times without any anomalies, just to familiarize yourself with what the corridor should look like. When the large yellow exit sign appears on the left, the game has officially begun. You can experience anomalies on the 8th loop, just to be on the safe side, turn around like you would with any other anomaly until you do a loop without an anomaly. At that point, you can continue forward and when you turn the corners, there should be a staircase which will lead you out of the underground and complete that run. Some of the anomalies are deadly and will reset your run; those have been clearly labelled.
For starters, follow the businessman around the corner and when he stops, stand in front of him and stare for five seconds. I'm not sure if this can be unlocked on the three practice loops, so if you try, and it doesn't pop, just wait for the yellow exit sign to appear and do it then. The man needs to have completely stopped for it to count. Blocking his way while he's walking and staring at him won't work.
1. There is a staircase at the end of the corridor instead of the usual left-hand turn. There's an achievement tied to this anomaly. Instead of turning around, simply walk up the stairs instead. Please note though that this will reset your run back to exit 0.

2. Two men in black suits will stand in the centre of the corridor.
3. (DEADLY) The businessman is walking faster than normal. The man is not only moving fast, he is actually chasing you and if he touches you, the run resets back to exit 0.
4. The businessman will stare at you as he walks by.
5. The businessman’s face is glitched out and partially transparent.
6. The businessman is much larger than normal.
7. The businessman is smiling.
8. This anomaly is one of the more subtle of the anomalies. There will be faces embossed in the yellow tiles on the floor of the corridor.
9. There are "No Smoking" signs plastered all over the right wall.
10. The lights are not in a straight line.
11. Another subtle anomaly, the posters on the left grow larger. You have to stare at them for a little bit to see if they grow. The posters are usually six and a half tiles tall, so if they’re over that then they’ve grown and that’s an anomaly. The screenshot shows the normal size of the poster on the left and the size they can grow to on the right.
12. The posters on the left have been replaced so all six are of the women’s eyes.
13. The second poster will have the cartoon woman with a creepy face with big eyes.
14. (DEADLY) Roughly a third to halfway down the corridor, you’ll hear rushing water and then waves of blood will crash around the corner and come down the corridor. This anomaly is deadly and will reset the run if caught in it. The water also travels around the corner so don't stop running until you hit the yellow exit sign.
15. The yellow exit 8 sign on the ceiling is upside down.
16. After walking by the yellow exit 8 sign on the ceiling, the back says ‘turn back turn back turn back’.
17. Black goo will start to leak out of the first air vent on the right wall.
18. The first of the three doors on the right is missing, and there are only two doors.
19. The first door on the right will shudder in the door frame repeatedly along with loud banging noises.
20. The middle door will be open a crack and someone will be looking through the gap. There's an achievement tied to this anomaly. You have to stare at the face in the gap for five seconds. The achievement wouldn't pop for me until I was wedged right up against the gap, but don't worry, there's no jump scare, the face simply continues to stare. Once the achievement has popped, you can turn around and treat it the same as any other anomaly.
21. The lights will completely turn off. This tends to happen toward the end part of the corridor near the two doors.
22. There will be additional yellow tiles on the ground across from the second door on the right.
23. The last poster has a creepy face.
24. There is a creepy face watermarked into the ceiling between the lights and across from the second vent.
25. The security camera poster slides along the wall with you as you walk by.
26. The eyes on the security camera poster move from side to side.
27. The security camera on the ceiling (above the security camera in operation poster) has a red light.
28. The last door on the right has a door knob in the centre of the door.
29. (DEADLY, sort of) The last door on the right will open as you walk by it. This probably goes without saying, but if you walk through the door it’ll reset your run.
30. The lights start to flicker. This tends to happen only toward the end of the corridor.
31. (DEADLY) There is a man hidden in the tiles at the end of the corridor. If you go close enough to him he will chase you. If he touches you, the run will reset.

So the achievement can seem a little confusing at first because in order to complete a run successfully, you need to do eight loops without being wrong, which seems like you've completed the game without a mistake. But in this case, what the game means by mistake is if your run resets because you missed an anomaly, you then have to complete the game again (i.e. reach Exit 8 and go up the stairs) and from there you have to reach Exit 8 successfully without a single reset. If you reset, then you have to complete the game to get back to the 'start' to try again. There is also the option of pausing and selecting Delete Play Data which of course would also put you in a fresh play state. I don’t know if that resets the tracker on the walk 10,000 steps achievement, and at this point I still had anomalies to collect, so I didn’t risk it. However, if you already have all the achievements apart from this one, then go for it.
Thankfully, once you’ve successfully completed a run there will be a sign in front of you that shows the amount of anomalies you have left to find. Once you’ve found 10, 20 and all 31 you unlock three achievements respectively.
At this point you’ll be very familiar with all the anomalies, so it’s time to go for the speed run achievement. We have to reach Exit 8 in under five minutes. It’s possible that while working through the other achievements you’ve already got it, but if not, simply follow the list as quickly and as carefully as you can.
While working on all this you’ll also be working toward your 10,000 steps. The good news is this achievement tracks so you’ll always be able to see how many you have left to do. The bad news is you'll likely have around half left to do which will take 45-60 minutes. You don't have to worry about turning away from anomalies or being reset by deadly anomalies. Just keep running the loops until the achievement pops.
